Matter of Balance
Falls prevention classes are available through this clinic. If you have concerns about falling or limit your physical activity to protect yourself, this class can help you. Ask your provider for more information.
Social Determinants of Health
Because CHAS recognizes that social conditions, such as housing, transportation, and access to food, often affect health, we created a partnership with Aging & Long Term Care to provide information and referral to our patients who are seeking these resources.
