mHealth

Mobile Health, or mHealth, is a program for several chronic health conditions that benefit from regular monitoring. With medical devices that connect by Bluetooth to the CHAS Health App, your health monitoring information is shared with your medical team.

mHealth | Remote Health Monitoring

CHAS health supports several mHealth remote patient monitoring programs. mHealth uses Bluetooth-connected devices to measure different biologic processes and gets this information to your care team without hassle—think of scales measuring your weight or glucometers measuring your blood sugar levels. This data is automatically uploaded into the CHAS Health App to show important trends that your care team will be able to use to form treatment plans.*

If measures are out of normal ranges or missing, your care team can proactively reach out to try and assist you to get back on the path to wellness. You will be better equipped to track your health, understand your health trends, and make decisions that are best for you.

*Data uploading for mHealth requires technical specifications and a reliable cellular signal which is provided by the patient, not by CHAS Health. See your Owner's Manual for details

Video Visits on the CHAS Health App

A video visit using the CHAS Health App is a convenient way to receive in-person care from the comfort of your own home.

More and more medical specialties at CHAS Health are incorporating video visits and other telehelath technologies into their care—all to provide you quality care more conveniently.

Chronic Obstructive Pulmonary Disease

There is a lot to think about when managing chronic obstructive pulmonary disease (COPD). Tracking your oxygen level every day can be a real challenge but is so important when managing COPD. The CHAS mHealth remote monitoring program is here to help. The CHAS Health App will send notification reminders to your cell phone and the "smart" pulse oximeter provided by CHAS uses cellular technology to track oxygen levels over time and sends that information to your care team. If problematic trends appear, the care team at CHAS can help identify and intervene before the trends become a crisis.

To track your oxygen level and ensure there are no significant drops in a short period of time, CHAS has provided you with a SmartMeter iPulse Oximeter to be used during your participation in the program and returned to CHAS in accordance with program requirements. CHAS has a support line for technical troubleshooting if you need additional help.

Congestive Heart Failure

There is a lot to think about when managing congestive heart failure (CHF). Tracking your weight every day can be a real challenge but is so important when managing CHF. The CHAS mHealth remote monitoring program is here to help. The CHAS Health App will send notification reminders to your cell phone and the "smart" scale provided by CHAS Health uses cellular technology to track weight over time and sends that information to your care team. If problematic trends appear, the care team at CHAS can help identify and intervene before those trends become a crisis.

HS4S Scale

To track your weight and ensure there are not dramatic gains in a short period of time, CHAS has provided you with a SmartMeter iScale Plus to be used during your participation in the program and returned to CHAS in accordance with program requirements. CHAS also has a support line for technical troubleshooting if you need additional help.

Hypertension

There is a lot to think about when managing hypertension. Tracking your blood pressure every day can be a real challenge but is so important when managing hypertension. The CHAS mHealth remote monitoring program is here to help. The CHAS Health App will send notification reminders to your cell phone and the “smart” blood pressure cuff provided by CHAS uses celluar technology to track blood pressure over time and sends that information to your care team. If problematic trends appear, the care team at CHAS can help identify and intervene before the trends become a crisis.

To track your blood pressure and ensure there are no significant drops in a short period of time, CHAS has provided you with a SmartMeter iBlood Pressure Classic - Connected Blood Pressure Monitor to be used during your participation in the program and returned to CHAS in accordance with the program requirements. CHAS also has a support line for technical troubleshooting if you need additional help.
