* We value the importance of continuing education in order to better serve our patients.  Therefore, please note, the third Wednesday and the last Wednesday of every month, we open at 9am.


At CHAS Health, we offer comprehensive services for patients of all ages.  Our North Central Clinic has a care team specializing in providing specialty care for individuals over 60 years of age.

We are excited to offer a clinic that meets the needs of senior members of our community. At CHAS Health North Central clinic, we have an easily accessible clinic, friendly providers and staff with a background in geriatric medicine. We are a full service clinic offering medical, behavioral health, and pharmacy services under one roof.

Primary Care - Our care provider team can help you and your family with all of your medical needs. Including:

Pharmacy

Our expert pharmacists work side by side with our providers. The collaborative approach provides wrap-around care for our patients. We have convenient, in-clinic pharmacies at all of our primary care sites for easy prescription pick-up. We also offer prescription auto-refill and mail delivery, check with your pharmacy for eligibility.  Additionally, our expert pharmacy staff offers anticoagulation services and diabetes education.

Behavioral Health

We offer psychiatry, medication management, and counseling for adults.

Matter of Balance

Falls prevention classes are available through this clinic. If you have concerns about falling or limit your physical activity to protect yourself, this class can help you. Ask your provider for more information. 

Social Determinants of Health

Because CHAS recognizes that social conditions, such as housing, transportation, and access to food, often affect health, we created a partnership with Aging & Long Term Care to provide information and referral to our patients who are seeking these resources.
